About the role:

The Western Aged Care Assessment (ACA) service plays a vital role within Western Health at Home, supporting older people with care needs to access appropriate Commonwealth-funded aged care services. Assessors undertake comprehensive clinical and non-clinical assessments, develop person-centred support plans, and coordinate referrals, reviews and assessment activities to ensure individuals receive the care that best meets their needs and preferences.

This role offers the opportunity to make a meaningful impact by enabling people to maintain their independence, wellbeing and quality of life through timely, holistic assessment and care planning. Working within a collaborative multidisciplinary team, the role contributes to Western Health’s mission by delivering high-quality, compassionate, person-centred care while supporting continuous learning, professional development and service excellence.

Key responsibilities:

• Complete person-centred aged care assessments using a wellness and reablement approach to support older people to access the services they need.
• Support older people and their families or representatives to navigate the My Aged Care system and ensure a smooth assessment and referral process.
• Apply relevant legislation, policies and professional judgement when assessing needs, developing support plans and making recommendations for Commonwealth-funded aged care services.
• Maintain accurate and timely documentation within the My Aged Care system, including assessments, support plans, reviews and referrals.
• Contribute to a positive team culture through ongoing learning, reflective practice and a commitment to delivering high-quality, client-centred care.

About you:

• Clinical Assessor Qualifications: Registered Nurse Division 1, Occupational Therapist, Physiotherapist, Speech Therapist or Social Worker.
• Current registration with AHPRA (all clinical assessors except Social Workers).
• Unregistered Social Workers must be eligible for membership of AASW.
• Demonstrated advanced knowledge/assessment/problem solving/care co-ordination skills (clinical).
• Experience working with elderly persons in a sub-acute, residential or community setting.
• Specific interest in Aged Care with demonstrated expertise working with clients from a range of culturally and linguistically diverse backgrounds.
• Current Victorian Driver’s License.
