About the Role
We have an opportunity for a Grade 1 Social Worker to join our CART Team on a part time (0.6 EFT), fixed term basis until March 2027. This position will be based in Cranbourne, but will require working at various Monash Health sites; covering the Greater Dandenong and Casey Cardinia catchments.
Your role as the Grade 1 Social Worker will be providing psychosocial assessment and intervention, coordination of care across the continuum and intervention for clients who have complex health issues/ needs. You will work in collaboration with the multidisciplinary team and be responsible for holding a clinical caseload which will involve assessment, care coordination, systems navigation, secondary consultation, service development and innovation.
You aim is to ensure that the clients and families have information and support, as required, to access a range of formal and informal supports. The program utilises an assertive outreach model in line the programs guidelines providing home visits as appropriate to clients residing in the Monash Health catchment.
About Us
The Primary Care Support Team, as part of the Community Assessment and Response Directorate offers a variety of programs that assist individuals to manage chronic health conditions, focusing on the assessment and delivery of client focused care.
The team consists of Allied Health Assistants, Care Coordinators, Community Development Workers, Counsellors, Diabetes Nurse Educators, Dietitians, Exercise Physiologists, Psychologists, Occupational Therapists, Physiotherapists, Social Workers, Recreational Therapists, Leisure and Lifestyle assistants, Nurses and Podiatrists. The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace client centred care, evidence-based practice and the Social Model of Health.
About You
You will be a dynamic, innovative and flexible clinical practitioner with a good understanding of the treatment and psycho-social issues facing individuals with chronic illness and their families. You will also have an understanding of the aged care sector, chronic health conditions and related services and referral pathways. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and hospital discharge planning.
You ensure that high quality Social Work services that place the client at the centre of are provided. Education, quality improvement and evidence based practices will be your key drivers. Due to your proven clinical experience, you will bring knowledge of healthcare reforms and initiatives and challenges that public healthcare is facing now and in the future.
Qualification & licence requirements include:
- Bachelor of Social Work or Masters Social Work Qualifying OR Masters of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
- You must be eligible for AASW membership including continuing professional education
- Current Driver’s Licence
