Position Overview We are seeking a full-time Marketing / Sales Representative to expand our referral network for Pooled Income Trust services. This is a true “road warrior” role, with most days spent traveling and conducting in-person outreach across New York, including Long Island, all five boroughs of NYC, Westchester, and Rockland. The ideal candidate is independent, highly organized, self-motivated, and excels at face-to-face relationship building and educating referral partners on complex financial tools that support Medicaid home care access. Success is measured by the volume and quality of referrals generated, as well as the strength and longevity of professional relationships developed.

Key Responsibilities

  • Serve as the primary outreach and relationship manager for referral partners, including elder law attorneys, geriatric care managers, social workers, discharge planners, home health agencies, home care providers, and organizations serving individuals with disabilities.
  • Conduct frequent in-person visits, educational presentations, and follow-up meetings with prospective and existing referral sources throughout the designated regions to build trust and clearly articulate the value of Pooled Income Trusts in enabling sustained home care eligibility.
  • Proactively build and maintain a consistent pipeline of potential referral sources through targeted research, cold outreach, strategic networking, and identification of new collaboration opportunities.
  • Educate referral partners in detail on Medicaid eligibility rules (especially income limits and spend-down for Community Medicaid/home care), Pooled Income Trust mechanics, allowable uses (including integration with home care expenses), and practical client benefits—ensuring partners feel fully equipped and confident to recommend our services.
  • Attend and participate in relevant networking events, professional association meetings, conferences, and community forums to raise awareness of the organization and its role in supporting home- and community-based care.
  • Coordinate closely with internal teams to document outreach activities, track referral progress and follow-ups, and ensure accurate reporting and alignment on referral development.
  • Represent the organization professionally at every interaction, demonstrating deep knowledge, credibility, empathy, and a unwavering client-first approach.

Qualifications

  • Required: Direct experience in home care or home health services (e.g., prior roles involving home care sales/outreach, geriatric care management, home health agency coordination, Medicaid home care navigation, community-based long-term care services, or closely related work). This experience is critical for understanding real-world client challenges with excess income and home care affordability, speaking credibly to referral partners (many of whom operate in home care), and effectively positioning Pooled Income Trusts as a practical solution.
  • Proven experience (3+ years preferred) in sales, marketing, business development, or professional outreach, ideally in healthcare, elder law, nonprofit social services, Medicaid planning, or related fields.
  • Strong familiarity with Pooled Income Trusts, Medicaid eligibility rules (particularly Community Medicaid/home care income limits and spend-down), or established relationships in the elder law, geriatric, disability services, or home care ecosystem is a significant advantage.
  • Exceptional interpersonal, verbal, and presentation skills, with proven ability to explain complex regulatory and financial topics clearly, persuasively, and accessibly to diverse professional audiences.
  • High self-motivation, strong organization, and time-management skills to excel in a fully mobile, field-based role with limited daily oversight.
  • Comfort and willingness to travel extensively (primarily by car) across Long Island, NYC’s five boroughs, Westchester, and Rockland, with reliable transportation and a valid driver's license.
  • Genuine passion for mission-driven work supporting seniors and individuals with disabilities, and commitment to ethical, compliant practices.

Compensation

  • Base salary: $75,000 - $100,000 annually, commensurate with experience.
  • Commission structure tied to referral performance.
